In an effort to protect our community, we will be asking you to take the following precautions:
Please DO NOT attend your appointment if:
Are sick with upper respiratory symptoms, including a runny nose, sore throat, cough, difficulty breathing and/or a fever. Please stay at home, self-isolate, drink fluids and rest. We will be happy to see you when you no longer display any symptoms.
Are self-monitoring after a potential exposure.
You or anyone within your household is currently waiting a COVID-19 test result.
Have travelled internationally in the past 14 days.
If you have any questions about whether you should reschedule your appointment because of illness or recent travel, please contact our office and we will help guide you.
Within 24 hours of your appointment you will receive a COVID-19 screening questionnaire and consent form to fill out. These will be sent through the booking system.
This screening must be filled out before arriving at the clinic for every appointment until further notice.
Screening must be completed successfully and honestly in order to go ahead with your appointment.
Wear a mask to your appointment. It must be worn at all times within the building.
ARRIVING TO YOUR APPOINTMENT
To maintain physical distancing, you are encouraged to arrive just prior to your appointment time to wash/sanitize hands.
Use hand sanitizer when entering the clinic, before you exit, and throughout your appointment as needed. You may also use the sink in the washroom to wash your hands with soap.
To avoid contamination, please dispose of any gloves that you’ve been wearing during your travels upon entry to the clinic.
